Plot summary
Alice wakes in the ruined White House after Wesker's betrayal, and the Red Queen appears with a task: reach the Hive beneath Raccoon City and release the airborne antivirus kept there. Umbrella plans to wait 48 hours until the last humans on Earth are gone. The virus lives in Alice's body, so she does not expect to survive the cure, but she accepts the mission. On the road, Umbrella's Dr. Isaacs captures her and drags her along with other prisoners; she breaks free and continues to Raccoon City, where survivors including Doc, Abigail, Christian, Cobalt, Razor and Claire Redfield have gathered. Isaacs' convoy approaches, with a horde of zombies behind it.

- Stuntwoman Olivia Jackson, Milla Jovovich's double, was severely injured on September 5, 2015 while filming when her motorcycle collided with a camera crane at high speed; she was riding without a helmet and the crane failed to move out of the way in time.
- Olivia Jackson sued the producers in 2019 for putting finances over safety, saying director Anderson ordered the camera crane raised one second later than in two rehearsals, and that she was misled about insurance covering injuries. She won the lawsuit.
- A second accident during filming, on December 3, killed crew member Ricardo Cornelius, who was crushed to death by one of the film's props, a US Army-issue Hummer, while on set.
- In China, the violence-intolerant State Administration of Press, Publication, Radio, Film and Television trimmed 7 minutes from the film before its release.
