Plot summary
Reggie has been crossing the desert alone, hunting the Tall Man. A motorist who stole his Barracuda gives him a ride, and Reggie throws him out of the car; spheres then kill the thief while Reggie fights them off. He comes to in a nursing home, in a wheelchair, visited by Mike, who tells him he has been diagnosed with dementia and treats his memories as dreams. In the dream, he meets Dawn, whose car has broken down, and drives her to a farmhouse kept by Demeter, a Bulgarian. Reggie recounts his history with the Tall Man and tries to win Dawn over, and she refuses. The Tall Man then arrives, and Reggie wakes in a hospital of the 1860s in the next bed to an elderly patient who talks with him.

- Don Coscarelli publicly denied that a new Phantasm film was in the works, but he and co-writer/director David Hartman told Entertainment Weekly the film had been shot secretly in and around Southern California during 2012 and 2013.
