Plot summary
Roy Bain, a former criminal, takes a job from Hugo Motherskille, a rich businessman and crime boss. Motherskille wants him to find Nicole, a prostitute Bain once loved, after she is kidnapped from her brothel. Bain begins investigating and follows the traces to Dr. Savary. Savary has produced a strange white powder that mutated humans in the tunnels and sewers under London want.

- Freur, the Welsh new wave group that scored the film, included Karl Hyde and Rick Smith, who took the film's title for their own band, Underworld, later known for "Born Slippy .NUXX" and the 1996 film Trainspotting.
- The Underworlders' mutations were practical make-up effects. Effects designer Peter Litten said some of George Pavlou's first concepts were too elaborate or grotesque to put on screen, so the designs were changed to keep recognisable human features and win sympathy for the mutants.
- Clive Barker interrupted work on his novel The Damnation Game to develop his earlier treatment for the film into a screenplay.
