Plot summary
In Rome in 1971, Father Brennan questions Father Harris about an occult conspiracy and receives a photograph of a baby marked with the name Scianna. Amid political protests, Margaret Daino, an American novice, takes up work at the Vizzardeli Orphanage, meeting Abbess Silva, Cardinal Lawrence, Father Gabriel, Sister Anjelica and Luz, a fellow novice who shares her room. Luz takes her to a disco, where Margaret dances with a man called Paolo and blacks out, waking the next day with no memory of the night. At the orphanage she grows close to Carlita, an orphan the sisters mistreat, and Father Brennan warns her that evil things will happen.

Did you know?!Facts and trivia about The First Omen (2024)
Can contain spoilers, watch the movie first.
- The Motion Picture Association initially registered The First Omen with an NC-17 rating over a single shot in the birthing sequence; producer Keith Levine said the team went back and forth with the ratings board five times and that avoiding the NC-17 made the film more intense.
- To portray the film's Rome orphanage, production designer Eve Stewart put three buildings together, including buildings in Villa Parisi and a farm in Procoio, because no single building had everything the filmmakers wanted; the basement was built on a soundstage.
- Costume designer Paco Delgado dressed the orphanage girls in 1940s and 1950s clothing even though the film is set in the 1970s, and designed light costumes that moved with the wind to match Arkasha Stevenson's vision of figures floating through the hallways.
