Plot summary
Years after returning from Burma, John Rambo keeps his late father's horse ranch in Arizona with his friend Maria Beltran and her granddaughter Gabriela, who looks on him as an uncle. Since his last battle, Rambo has been troubled by PTSD, and he has dug a network of tunnels beneath the ranch. Gabriela is told that her father Manuel is in Mexico. Rambo says that pursuing the man would only hurt her, but Gabriela drives south anyway. Manuel tells her he never wanted a child, and at a club she is drugged and taken by a cartel's men. Maria learns that Gabriela has disappeared.

Can contain spoilers, watch the movie first.
- For a while the fifth Rambo was not a Mexico story at all. After Millennium greenlit it in 2009, Sylvester Stallone retitled it Rambo V: The Savage Hunt and rebuilt the plot around Rambo leading a special forces team after a genetically engineered creature, loosely based on James Byron Huggins' novel Hunter, before reverting to the kidnap story.
- In 2015 Stallone and David Morrell, who created Rambo in the novel First Blood, wrote a new version together, aiming for what Morrell called a really emotional, powerful story. The producers preferred the human trafficking plot and their script was abandoned.
