Plot summary
John Wick recovers underground in New York with the Bowery King after Winston shot him off the Continental roof. The High Table answers his plan for vengeance by sending the Marquis Vincent Bisset de Gramont, who has unlimited resources and authority. In New York, the Marquis summons Winston and Charon, removes Winston from his managerial duties, declares him excommunicado, and destroys the New York Continental. He also forces Caine, a blind retired assassin and John's old friend, to hunt John by threatening Caine's daughter. John takes refuge at the Osaka Continental, run by his friend Shimazu Koji. Caine and Chidi arrive with High Table enforcers to investigate. Akira, Koji's daughter, gets the hotel's people out before the High Table shuts the building down, and a fight breaks out.

- Several French crew, including the location manager, described filming in Paris as "hell"; a two-week delay filming in Germany left Stahelski less time to scout Paris, so he improvised and changed locations at the last minute.
- Stahelski described an early cut as "screwed" because the expanded worldbuilding and the new characters took 225 minutes of screen time, and Nathan Orloff edited the film substantially to bring the runtime down to 169 minutes.
- The chase-shootout that appears to circle the Arc de Triomphe was not entirely shot at the monument: it was finished at a disused airport near Berlin.
- Rina Sawayama made her feature film debut in the picture, chosen because the role needed someone who could do choreography after Stahelski saw her music videos for "XS" and "Bad Friend", which feature dance and fighting.

An alternate ending in which John visits his grave was filmed but abandoned after it received a "violent reaction" from test audiences.- Hiroyuki Sanada, cast alongside Laurence Fishburne, Donnie Yen, Bill Skarsgård, Shamier Anderson and Scott Adkins, had first been courted to play Zero in Chapter 3 but dropped out with a torn Achilles tendon injury.
- The name of the character Killa Harkan is a reference to the Berlin-based Turkish rapper Killa Hakan, rather than an invention of the screenwriters.
