Plot summary
In West Berlin during the German Autumn of 1977, Susie Bannion, raised in a Mennonite family in Ohio, is admitted to the Markos Dance Academy. The school is unsettled by the disappearance of Patricia Hingle, a student who had told her psychotherapist, Josef Klemperer, that a coven of witches controls the academy; the journals she left in his office describe three witches older than Christianity. Klemperer, who first dismissed her, grows wary of the school. When a Soviet dancer, Olga, storms out of rehearsal, she is trapped in a room lined with mirrors while Susie dances. The matrons then vote on their new leader, choosing Mother Markos over Madame Blanc. Susie befriends Sara, becomes Madame Blanc's protégée, and wins the lead in the academy's coming piece, Volk.

Tilda Swinton played Dr. Josef Klemperer, but the film credited the role to a fictitious actor named "Lutz Ebersdorf"; the filmmakers maintained that Ebersdorf was a real psychoanalyst until a month after the film's premiere.- The estate of artist Ana Mendieta sued Amazon Studios for copyright infringement, alleging that two images in the film's teaser trailer were plagiarized from her work, a woman's rope-bound hands on a white table and a red body silhouette on a bedsheet.
- Guadagnino offered the project to director David Gordon Green, who cast Isabelle Huppert, Janet McTeer and Isabelle Fuhrman in his own Suspiria remake before financing conflicts scrapped it.
- Radiohead singer Thom Yorke wrote the score, his first feature film soundtrack, only after initially refusing the offer and accepting following months of requests from Guadagnino.
- For the scene in which Olga's limbs and jaw are crushed, a prosthetic arm, leg, ribs and protruding dental cast were made for actress Elena Fokina, a professional dancer and contortionist who performed most of the contortions herself; Coulier took the idea from Ronny Cox's self-dislocated shoulder in Deliverance.
- For the climactic sabbath scene, a loggia in the hotel had its arches filled in and covered with braided artificial hair; Weinberg said using hair was Guadagnino's idea and that the wall's texture was conceived as the hair of victims.
