Plot summary
The infected outnumber the healthy, and the illness has spread across the planet. At a military base, Doctor Greer briefs Lauren, a doctor, before sending her out with an extraction team. She is to test anyone they find so that no infected person is brought back inside. Greer shows her how the illness progresses, up to level 5, extreme aggression. Her teammates are Wheeler, who drives, Denise, who navigates, and Gunner; their destination is a school where uninfected people have barricaded themselves. Denise tells her that Gunner's wife was on the earlier team sent to the school and has not been seen since. Lauren's daughter Megan answers her call from the family house in the Los Angeles suburbs, and Lauren promises to come for her.

- The movie's first-person point of view was achieved by using two camera operators, one man and one woman, who alternated shooting scenes, swapping in whenever the on-screen viewpoint belonged to a character of their own sex.
- Director John Suits had been a gamer as a kid but had no time to play video games for research on this project, so he worked out the film's violent kill scenes by talking to gamer friends, watching YouTube clips, and consulting the stunt coordinator.
