Plot summary
For four hundred years Jacob Kell has hunted the people Connor MacLeod cares about. In Glenfinnan in 1555, Kell, then a priest, had Connor's mother executed for witchcraft, and Connor killed him; Kell returned as an immortal, ignoring the rules of the Game by gathering lesser immortals as disciples who overpower opponents so he can take their heads. He has taken more than six hundred heads. Duncan MacLeod, Connor's kinsman, goes looking for him after Kell's assault on the Sanctuary where Connor had taken refuge. Kate, Duncan's wife of two hundred years earlier, blames him for making her immortal against her will, and now rides with Kell's followers.

- The trailers feature several shots of Kell using mystical abilities, such as stopping a sword in midair with a force field and cloning himself. None of that footage made it into any released version, and Douglas Aarniokoski said the shots were never intended for a finished cut and were filmed exclusively for the trailer.
- Endgame's theatrical distributor, Dimension Films, demanded cuts to give the film a faster pace. The deleted footage contained exposition necessary to understanding the Highlander universe, resulting in a theatrical cut that was criticized for being incomprehensible to audiences unfamiliar with the earlier films.
- Donnie Yen served as the film's martial arts choreographer and also played the role of Jin Ke, who was based on a real historical figure: Jing Ke, renowned for his failed assassination of the Chinese emperor Qin Shi Huang.
