Plot summary
Mia is a heroin addict, and her brother David brings her to the family's holiday cabin in the woods so she can go through withdrawal. His girlfriend Natalie and their friends Olivia, a nurse, and Eric, a high school teacher, come along. Olivia knows this is not Mia's first attempt, and Eric recalls that the last one ended in a relapse and a legal death before Mia was revived. The cabin has been broken into, and in the cellar David finds rotting animal corpses, a shotgun and an old book, the Naturom Demonto. Eric recites an incantation from its pages, and something malevolent answers. Out in the woods, Mia sees a demonic double of herself.

- The film first received an NC-17 rating, which prompted cuts so it could obtain the contractually obligated R rating; director Fede Álvarez announced this on Twitter on January 28, 2013.
- Director Fede Álvarez said the film used no CGI except for touch-ups: everything on screen was real, done through researched magic tricks and illusion tricks over a demanding 70-day night shoot.
- Sam Raimi and Bruce Campbell had planned an Evil Dead remake for years, but in 2009 Campbell said the proposed remake was "going nowhere" and had fizzled because of extremely negative fan reaction.
- Lily Collins was initially scheduled to play the lead female role of Mia, but she dropped out in January 2012 and Jane Levy replaced her the next month.
- Diablo Cody was brought in to doctor the script Fede Álvarez and Rodo Sayagues co-wrote, in an effort to Americanize the dialogue because English was not the writers' first language.
