Plot summary
FBI agents Chester Desmond and Sam Stanley arrive in Deer Meadow, Washington, where the body of night-shift waitress Teresa Banks has been found, and the locals and the police give them nothing. Searching Teresa's home, the agents see a photograph of her wearing a ring that is no longer there; Desmond finds the ring, and then he is gone. In Philadelphia, Gordon Cole, Dale Cooper and Albert Rosenfield hear from Phillip Jeffries, an agent long missing, who speaks of mysterious spirits and then vanishes himself. A year later in Twin Peaks, homecoming queen Laura Palmer dates drug dealer Bobby Briggs, makes money as a prostitute to pay for cocaine, and is haunted by Bob, a spirit who has abused her since she was twelve.

- Kyle MacLachlan agreed to return for the film on condition that he appear in only five days of shooting, which forced Lynch and Engels to rewrite the first act, originally about Cooper investigating Teresa Banks' murder.
- David Bowie shot his scenes in four or five days because Tin Machine needed to rehearse for an upcoming tour. He disliked his Southern accent and asked Lynch and Frost to overdub his lines when they reused archive footage from the film in Twin Peaks: The Return.
- New Line backed out of including the deleted scenes on its 2002 Special Edition DVD, with Lynch saying the studio balked at his $100,000 fee to edit, mix and color grade the footage; MK2, which held the rights, never released them either, and Lynch blamed the 2008 financial crisis.
- Director David Lynch had a hernia for the entire shoot, having injured himself by laughing too hard at something funny Angelo Badalamenti did.
- Twin Peaks co-creator Mark Frost took an executive producer credit but had no involvement: he wanted a sequel because he felt the audience wanted the story to go forward, while Lynch wanted a prequel, saying he was in love with Laura Palmer's contradictions.
- Sherilyn Fenn, who played Audrey Horne, did not return for the film; in 1995 she said she didn't want to return because she was 'extremely disappointed in the way the second season got off track'. Her absence had first been attributed to scheduling conflicts.
