Plot summary
King George III is devoted to his wife, Queen Charlotte, and to their fifteen children, and he takes a close interest in the workings of his government. In 1788 his memory begins to fail, he talks without stopping, and his behavior turns erratic, and the physicians of the day have no answer for it. His eldest son, the Prince of Wales, chafes under his father's criticism and wants more freedom for himself. Prime Minister William Pitt the Younger heads the government, and the opposition, led by Charles James Fox, presses for a regency.

- Director Nicholas Hytner changed the title from the play's The Madness of George III to The Madness of King George for American audiences, to clarify George III's royalty. Asked about the popular story that it was to stop people assuming they had missed "I" and "II", he laughed: "That's not totally untrue."

Two text panels at the end of the film note that the color of the King's urine suggests that he was suffering from porphyria, adding that the disease is "periodic, unpredictable and hereditary."
