Plot summary
Frank Kitchen works as a professional hit man. A disgraced plastic surgeon has lost her medical license and performs illegal experiments. When gangsters double-cross Frank, the surgeon takes him and subjects him to gender reassignment surgery, turning him into a woman against his will. Horrified by his new body, Frank falls apart. In his room he finds hormones and a tape recorder with a message from the surgeon urging him to start over. He leaves the hotel and contacts Johnnie, a woman he had hooked up with before, and she lets him stay with her while he recovers.

- The transgender community was largely disappointed by the film's forced gender reassignment surgery plot, and a boycott was created. Director Walter Hill responded that he wouldn't make a movie that hurt transgender people, though he understood the concern and called the film lurid, maybe lowbrow, but not offensive.
- Michelle Rodriguez spent four hours each day in male prosthetics and makeup to play hitman Frank Kitchen, and she chose for her character to have the largest penis available.
- Walter Hill optioned Denis Hamill's script with his own money about ten years after he first read it, could not make a different version work, and set the project aside for fifteen more years before re-optioning it.
- Producer Saïd Ben Saïd agreed to invest on just two conditions, Walter Hill said: the film had to be made very cheaply, and there had to be name value in the cast.
