Plot summary
In January 2007, Leena Klammer gets out of the Saarne Institute, the Estonian psychiatric facility where she is a patient, and hides in the car of Anna, an art therapist, whom she kills at home. She is 30 but looks about 10. Going through records of missing American girls, she picks Esther Albright, a rich family's daughter who disappeared in May 2003, since she matches a projection of how the girl would look grown up. She tells a Russian police officer she is Esther and that her parents live in the United States. In Darien, Connecticut, Detective Donnan brings Allen Albright, an artist, and his wife Tricia word that their daughter is alive. Tricia flies to Moscow and comes back with the girl, who grows fixated on Allen and works to push his wife away.

- Isabelle Fuhrman pushed for Orphan: First Kill after an Indiana couple charged with abandoning their foster child said they had been inspired by Orphan. Fuhrman said many people contacted her about the similarities between the case and the film.
- The production crew used makeup and forced perspective shots so Isabelle Fuhrman could play Esther again without CGI, and two female child actors served as her body doubles.
- Isabelle Fuhrman worked with the writer to develop the prequel intending to produce and cameo while a new actress played Esther, but the two instead decided she would reprise the role herself.
