Plot summary
Beth, Whitney and Lorna are American art students in Rome. Axelle, a nude model they sketch, talks them into a spa trip to Slovakia, and the four travel by train. On the way, a passenger accosts Beth, and Lorna loses her iPod, which Axelle gets back. The group checks into a hostel in the village, where a harvest festival is under way that night. The desk clerk uploads copies of their passports to the Elite Hunting Club's auction, where wealthy clients pay for the chance to torture and kill. Two American businessmen, Todd and Stuart, each buy a victim and head for Slovakia.

- The poster was conceived by Lionsgate marketing executive Tim Palen as a close-up picture of boar meat, and the Motion Picture Association of America only approved it when Palen showed them the butcher shop receipts for the meat.
- Matarazzo's elaborate murder sequence required the actress to hang upside down, nude, with her hands chained to her back, and she performed the scene herself, hung upside down in five-minute intervals over a period of two days.
- Phillips said her torture sequence, which entails her being scalped by a power saw, required around forty-five setups, and told a 2007 interview that she loved her job but "we went into places that I didn't know existed".
- Kňažko joked that "the fact that Sasha was Russian was one of the reasons I accepted this role", adding that "we Slovaks are still a little bit angry over the occupation of Czechoslovakia by the Soviet army".
- On October 8, 2007 the film was cited in the House of Commons as an example of stills that could be illegal under a proposed law on "extreme pornography", with MP Charles Walker admitting he had never seen it but was "assured by trusted sources".
- The "German Extended Version", in which Lorna's torture and death scene is still not shown completely, was banned in Germany after a Munich court ruled the uncut release punishable by law; the ban was lifted in 2022.
- Eli Roth sought Jared Harris for a role and wanted to meet him about it, but Harris watched the first film to familiarise himself with the franchise, found himself unnerved by the depravity of the subject matter and declined further involvement.
- Before its theatrical release a workprint of the film leaked on the Internet, and one publication at the time called it the "most pirated film ever", which Roth suggested was a factor in the film's box office returns.
