Plot summary
Six months after a welterweight championship bout in Las Vegas ended with the death of his opponent Jonny Sutherland, Wes Baylor lives in self-imposed exile in Thailand and fights in underground clubs. Jonah Aldrich watches him fight and offers him a million dollars for a private match in Myanmar. Baylor accepts, and on arrival he is told that Aldrich has organized a hunt with Baylor as the prey. The hunters include Sofia, the bullfighter Esparto, the video game designer Landon, Maduka, Jacob Zimling and his son Tobias, and Aldrich's right-hand man Madden, with corrupt Myanmar Army soldiers along. Baylor is given a water bottle and a money belt.

- Reiné had Scott Adkins in mind to play Baylor before the screenplay was even finished, but Adkins had to get clearance from Disney because he had already signed for the Marvel Studios film Doctor Strange. Disney let him skip training sessions for it so he could do both.
- Adkins had been a fan of the first film since winning a contest as a teenager to attend an early screening of it, yet he turned the sequel down at first: he had already been associated with Van Damme on several projects and was eager to stand out on his own.
- John Woo, director of the original, was supposed to take a producer credit, but he went unmentioned in Universal's wrap statement and in the finished film. The sequel still borrowed his style with doves, a mexican standoff, a motorcycle gunfight and the helicopter boat chase he could not fit into the original.
- Adkins and Reiné wanted Robert Knepper to play the brother of Emil Fouchon, the 1993 film's villain played by Lance Henriksen, but Universal vetoed it and insisted on a standalone. Reiné still gave Knepper's character the single-bullet pistol Fouchon used.
- The finale was to be staged on a rooftop in Bangkok's Chinatown until the location fell through. Producer Lowenstein had earlier scouted a picturesque bridge on the River Kwai and suggested moving the climactic showdown there.
