Plot summary
Eve Macarro is the daughter of Javier, an assassin of the Ruska Roma, and of a mother who belongs to the Cult. Her father takes her out of the Cult as a child, and the raid that follows ends his life. Winston Scott, who runs the New York Continental, places her with the Ruska Roma, a company of dancers that hides a guild of killers, and the Director takes her in. Eve spends twelve years becoming a ballerina, an assassin and a bodyguard, and earns the title Kikimora. When a Cultist bearing the mark of her father's killers attacks her on a mission, she asks the Director about the Cult, and the Director forbids her from pursuing it.

- Producer Chad Stahelski reportedly oversaw two to three months of reshoots on Ballerina, refilming most of the movie with director Len Wiseman not on set. Both men denied this, saying it was only two weeks of additional photography that Stahelski oversaw with Wiseman.
- Lance Reddick's role as Charon in Ballerina was his final appearance as the character and his final on-screen appearance, released posthumously two years after his death in March 2023.
- Shay Hatten wrote his spec script Ballerina as a standalone action-thriller about a ballerina-assassin, inspired by the trailer for John Wick: Chapter 2; when Lionsgate bought it in July 2017, they had him rewrite it as part of the John Wick universe.
- At Ana de Armas's insistence that a female screenwriter be involved, Emerald Fennell was hired in July 2022 to take a pass on the Ballerina script.
- Marco Beltrami and Anna Drubich were hired in April 2023 to score Ballerina, but were replaced in September 2024 by John Wick regulars Tyler Bates and Joel J. Richard.
