Plot summary
Rose Phipps practices ophthalmology and is living apart from her husband, a school teacher. The couple's two-year-old son died in a fall from a window, and she grieves him. Tommaso Pensara paints, and he has received news that he will lose his sight. Simon Colon is twenty and works in a photo shop; he lives with his mother Marta, who drinks. The three are strangers to one another and travel the 1 and 9 subway line in New York. Simon borrows cameras and follows Rose to photograph her scarf.

Simon's incestuous relationship with his mother in the film was drawn from De Villa's own complicated family history; the director insisted he and his mother never had such a relationship, saying he grew up essentially without a father and made the character's bond complex on purpose.- De Villa said the script was much darker in earlier drafts but was toned down for financial reasons: the mother-and-son scenes were more explicit, and what Heather Graham's and Victor Rasuk's characters did was darker, "more than just the spanking."
- The film was originally titled 1/9 after the subway lines, with a scene of Rose pursuing a stalker in the subway; after Sundance accepted it, De Villa was asked for a more mainstream title, and the scene was reworked after the "cat and mouse" sequence in The French Connection.
- Financing Adrift in Manhattan took two years and, in De Villa's words, "a bunch" of producers; once the cast was attached, producer Steve Brown got involved and, De Villa said, "magically raised the money."
