Plot summary
A hospice nurse named Reuben persuades one of his patients, Sam, to join a group trip into Manhattan for a marionette show. Sam, a terminally ill cancer patient, brings along her cat, Frodo. In the city, meteor-like objects fall from the sky, and alien creatures that hunt by sound swarm the streets. An explosion knocks Sam out. She wakes inside the puppet theater with Frodo and other survivors nearby, among them Henri, who signals her to stay quiet. Military helicopters broadcast warnings telling civilians to remain silent and hidden until rescuers arrive. Sam watches fighter jets destroy the bridges out of Manhattan so the creatures cannot leave the island.

- Lupita Nyong'o asked director Sarnoski to change her character's cat in A Quiet Place: Day One because she was afraid of cats, and overcame the fear through exposure therapy; by the end of production she had adopted a cat of her own.
- Jeff Nichols was set to write and direct A Quiet Place: Day One and had completed a script, but he dropped out in October 2021 to focus on another Paramount project; Michael Sarnoski replaced him as writer and director in January 2022.
- Paramount executives wanted Sam's cat Frodo to be computer-generated, but Michael Sarnoski's insistence on a real cat changed their minds; Frodo was played by two cats named Nico and Schnitzel.
- The script had Frodo hissing and arching his back, but animal handlers could not train the cats to do it, so the filmmakers removed those elements rather than scare the cats or use CGI.
- Denis O'Hare was cast in A Quiet Place: Day One in an undisclosed role, but his character was cut from the final film.
- The backlot at Warner Bros. Studios, Leavesden was used to recreate four New York City blocks, each two stories high, including Mott Street in Chinatown and parts of Harlem, the Lower East Side and the Upper East Side.
- The exterior of Patsy's Pizzeria was rebuilt for the film's set from extensive photography of the real New York restaurant, and the interiors of Loetta's Jazz Club were shot at the George Tavern in Stepney.
