Plot summary
The Fingers, a Satan-worshiping gang under Sir Lord Jimmy Crystal, take in Spike, a boy on the mainland. To be initiated he must fight a death match, after which Sir Jimmy renames him Jimmy to match the other Fingers, among them the empathetic Jimmy Ink and the sadistic Jimmima. Dr Ian Kelson coats his skin in iodine solution to keep the virus out, and he keeps adding to the Bone Temple, his ossuary for the Rage Virus dead. Samson, the imposing Alpha Infected, visits him again and again, and over those visits the two of them form a human connection.

- To play the Alpha infected Samson, Chi Lewis-Parry wore a full-body prosthetic suit. Each suit took seven artists six to eight hours to apply and could only be used once, so the process had to be repeated more than 25 times during filming.
- The scene in which Dr Ian Kelson and Samson dance to Duran Duran's Rio was not in the script at all; it was improvised on set during filming.
- The Bone Temple set was built in Redmire, North Yorkshire, and the opening scene's abandoned leisure center was shot at the Richard Dunn Sports Centre in Bradford, which has been closed since 2019.
- Nia DaCosta, protective of the franchise's legacy, pitched the film as a personal, idiosyncratic work rather than a copy of Danny Boyle's style, and asked writer Alex Garland to put more infected in the script.
- Where the previous film was shot on iPhone 15 Pro cameras, The Bone Temple switched to the Arri Alexa 35 digital camera.
