Plot summary
The Parkers buy supplies at a local goods store for a period of ritual fasting. Mrs. Parker notices a poster about a missing teenage girl, begins bleeding from the mouth, and drowns in a water-logged ditch. Her husband Frank does not come to identify the body; he sends his daughters, Rose and Iris. Doc Barrow, who delivered the family's young son Rory, carries out the autopsy the state requires and finds evidence of a prion disease. Rose and Iris argue over whether they are ready to take on their mother's religious duties.

- Director Jim Mickle did not originally want to direct the remake because he dislikes American remakes of foreign horror films; after speaking with Jorge Michel Grau, he and Damici decided they could put their own spin on it.
- Because the film's dark subject matter caused issues with its younger cast, Mickle consulted Jack Gore's parents, and they agreed that Gore should know only his own scenes.
