Plot summary
Mark Renton, 46 and two decades off heroin, has a heart attack in an Amsterdam gym, faces divorce and redundancy, and decides to return to Edinburgh. Daniel "Spud" Murphy has returned to heroin after he and his wife Gail separated and he lost custody of their son Fergus. Simon "Sick Boy" Williamson no longer uses heroin but takes cocaine, runs a failing pub he inherited, and works blackmail schemes with his girlfriend, Veronika. Renton stops Spud from a suicide attempt and offers to help him get clean. Simon attacks Renton at the pub over money from years ago; Veronika intervenes, and Renton gives Simon his old share back. Simon and Veronika plan a brothel disguised as a sauna above the pub. Francis "Franco" Begbie escapes from prison and returns to his wife June and their son, Frank Jr.

- Danny Boyle first declared in 2007 that he wanted to make a sequel to Trainspotting set nine years after the original, based on Irvine Welsh's Porno, but waited until the original cast had aged visibly enough to play the characters again, wanting them to "look ravaged by the passing of time".
- A script for the sequel had reportedly been written about 10 years earlier but was scrapped so that the original cast would agree to return, and John Hodge instead wrote an original screenplay rather than a strict adaptation of Irvine Welsh's Porno.
