Plot summary
John Kramer learns his brain cancer is terminal. At a support group meeting he befriends Henry Kessler, who says a Norwegian experimental treatment cured him. John gets in touch with the doctor's daughter, Cecilia Pederson, who sends him to her clinic outside Mexico City. There he meets Cecilia and her team, among them anesthesiologist Mateo, nurse Valentina, and Dr. Cortez, along with patients Gabriela and Parker and Carlos, the caretaker's young son. After the procedure, Cecilia tells him he no longer has cancer. He buys a gift for Gabriela, then goes back to the clinic and finds it empty. A video of a faked operation and, under his bandages, no scar at all show him what the treatment really was.

- Because the traps in Saw X were more complex than in earlier films, filming was split into two parts of three weeks each, in November and January, and the crew spent Christmas working out prosthetics and machines.
- The filmmakers considered using visual effects to de-age Tobin Bell and Shawnee Smith due to the 17-year gap since their characters died in Saw III, but decided against it; director Kevin Greutert felt de-aging puts distance between the character and the audience, and Bell and Smith have emotional close-ups that should not be altered.
- During an editing session for Saw X, first assistant editor Steve Forner had local police called on him by neighbors who thought someone was being tortured to death inside; the cops laughed when Forner explained he was just working on a movie.
- A parody of the Nicole Kidman AMC Theatres commercial, featuring a remote-controlled Billy the Puppet on a tricycle, was published to YouTube and Lionsgate's social media on September 13, 2023, but was pulled from Lionsgate's accounts after AMC Theatres issued a cease and desist order.
- Tobin Bell helped with the script and post-production of Saw X, adding dialogue that was worked into the story after filming ended.
- Hannah Emily Anderson signed on to play Eleanor Bonneville again in two more films after Jigsaw, but those films never happened.
