Plot summary
Pat and Bruce Gardner live in the Chicago high-rise that Bruce manages, and they know nothing of what their niece Carol Anne Freeling's family went through, only that her father was caught up in a bad land deal. Carol Anne has been sent from California to stay with them, and she is told the reason is a special school for gifted children who have emotional problems. Bruce's daughter Donna lives with them. Dr. Seaton, the teacher and psychiatrist who questions Carol Anne, treats her account as a delusion and keeps pressing her to repeat it. The sessions allow the spirit of Reverend Kane to find her, and he begins appearing in the building's mirrors. Tangina Barrons, who has dealt with Kane before, learns he has located Carol Anne and travels to Chicago to shield her.

- Heather O'Rourke, who played Carol Anne Freeling, died at the age of 12, four months before the film's release, making Poltergeist III her final performance; her death caused marketing complications for the studio, Metro-Goldwyn-Mayer, which did not want to exploit the tragedy.
- On May 13, 1987, while filming the snow-covered parking garage sequence in Oakbrook Terrace's Mid America Plaza, a special effects explosion set six foam-covered cars on fire; a maintenance worker and two firefighters were injured, and the building damage was estimated at $250,000.
- After O'Rourke's death, director Gary Sherman did not want to complete the film, but pressure from the studio prevailed and the entire ending was re-shot in March 1988 using seven-year-old child actress Heather Holty as a body double for O'Rourke.
- Craig T. Nelson was asked to play Steve Freeling again in the third film but declined, saying two was enough.
- One of Nancy Allen's earliest scenes was the finale, in which she and Tom Skerritt, as Pat and Bruce, scale the building on a window cleaning platform; it was filmed on a real platform about 100 stories high.
- Recurrent budget restrictions during the shoot forced director Gary Sherman to make unexpected alterations to the original screenplay he had written with co-writer Taggert.
