Plot summary
On October 30, 1988, Michael Myers has lain comatose at the Ridgemont Federal Sanitarium for ten years, since the night a fire ended his first spree in Haddonfield. An ambulance carries him toward Smith's Grove, and when the crew mentions his young niece, Jamie Lloyd, he wakes and kills them. The wrecked ambulance is found in a river, and Dr. Sam Loomis, who survived the same fire, sets out after him. In Haddonfield, Jamie lives with the Carruthers family and is bullied at school as the niece of the boogeyman. Her foster parents leave for a business meeting, so Rachel, their teenage daughter, cancels a date with Brady to look after her. Loomis warns Sheriff Ben Meeker that Michael is coming back to town.

Producers first asked Jamie Lee Curtis to return as Laurie Strode, but she declined to continue with the series (though she came back for the seventh Halloween film), so the writers killed the character off, a death only briefly explained in the movie.- Melissa Joan Hart auditioned for the role of Jamie Lloyd, Michael Myers' young niece, but the part went to Danielle Harris, who won it after auditioning in New York and had previously had a recurring role on the soap opera One Life to Live.
- Writer Alan B. McElroy was hired only eleven days before a Writers Guild strike deadline, forcing him to develop the concept, pitch the story and deliver a finished script for Halloween 4 in that short window.
- During filming, gaffer Garlan Wilde slit his wrists after dropping a light and had to be rushed to the hospital, and actress Ellie Cornell cut her stomach open on a nail while sliding down a roof for the rooftop scene but kept filming despite losing a significant amount of blood.
- Actor George P. Wilbur wore hockey pads under Michael Myers' costume to look more physically imposing and was often shot off-center or in mirror reflections so audiences saw him only "in pieces," and he regularly removed the mask in front of young co-star Danielle Harris to reassure her it was not real.
- A mistaken mask swap made it into the finished film: director Dwight H. Little believes a tired crew member grabbed the wrong, unaltered mask from the prop area for one shot, nobody caught the error on set, and it was left uncorrected because there was no time to reshoot.
