Plot summary
After his previous caretakers die in accidents, Mikey is placed with Neil and Rachel Trenton, a couple who know nothing of his history. He settles into their house, takes charge of their aquarium fish, and befriends Ben, the boy next door. Ben's older sister Jessie is home from college and dating a student named David, and Mikey becomes fixated on her. At school his teacher, Shawn Gilder, grows concerned about him, from a quarrel over the class marble jar to a drawing she finds disturbing. She and the principal, Mr. Jenkins, urge that Mikey see a therapist, though Rachel and Neil insist they will handle him themselves.

- British child psychologists who reviewed Mikey for the BBFC judged it potentially harmful to impressionable young viewers, specifically citing the two children who killed toddler James Bulger, and found that cuts to its most violent scenes did little to change that assessment.
- The BBFC rejected Mikey outright in 1996 despite having passed the film's trailer with an 18 rating back in November 1992, months before the Bulger murder; the ban remained in place until the film was finally unbanned and rated 15 for home viewing on March 20, 2025.
