Plot summary
Cuesta Verde, the neighborhood where the Freelings used to live, is now an archaeological dig, and the excavation uncovers a cave. Tangina Barrons, a psychic, learns of it and tells her friend Taylor, a Native American shaman. Taylor inspects the cave himself and concludes that the spirit of Rev. Henry Kane is after Carol Anne Freeling, so he goes to protect her. The Freelings have moved into a house belonging to Jess, Diane's mother, who is clairvoyant and believes Diane and Carol Anne share her gift. Taylor reaches the house during Kane's first attack on the family. Since every television set has been removed from the home, Kane's minions come through Carol Anne's toy telephone. The family gets out quickly, and Taylor tells them that Kane would find them again wherever they went, so they go back inside.

- A college-set sequence for Dana, the Freelings' eldest daughter, was scrapped from the sequel because Dominique Dunne, who played her in the first film, had been murdered in 1982.
- Voice actor Corey Burton was brought in during post-production to loop some of actor Julian Beck's lines, after Beck's death.
- MGM hired Swiss surrealist H. R. Giger to design the "Great Beast" manifestation of Kane, but because he did not want to leave Zurich for long periods, his colleague Cornelius De Fries was hired to represent him at the studio.
- The film's television advertisement gave it the catchphrase "They're ba-aaack", which anecdotal evidence credits with popularizing the phrase even decades later.
- Beatrice Straight was asked to reprise the role of Dr. Martha Lesh from the first film, but she was ill and not willing to work at the time.
- Richard Lawson was also asked to reprise Dr. Ryan Mitchell, but his schedule clashed with MGM because he was busy filming the TV movie Under the Influence (1986) with Andy Griffith and Keanu Reeves.
- Scenes that had already appeared in press stills and promotional posters were cut from the finished film, among them Tangina Barrons confronting Reverend Henry Kane at the door and Steven and Diane Freeling seeing a flying toaster at breakfast.
