Plot summary
Ellen's prayer as a lonely girl for a companion reached something called the Nosferatu, and a psychic link formed between them. Years later, in 1838, she is married to Thomas Hutter and living in Wisburg, Germany. Thomas's employer, Herr Knock, sends him to Transylvania to close the sale of a manor to Count Orlok, and Ellen stays with his friend Friedrich Harding, Friedrich's pregnant wife Anna and their daughters. The locals in a Transylvanian hamlet shun him when he asks about Castle Orlok, and he continues on foot. At home, Ellen begins to sleepwalk and suffer seizures, and her doctor, Wilhelm Sievers, treats her with modern medicine.

- Bill Skarsgård lost a significant amount of weight to play Count Orlok and refused to have his voice digitally modulated, instead working with Icelandic opera singer Ásgerður Júníusdóttir to lower his range and add Mongolian throat singing; his prosthetic makeup took up to six hours a day.
- Casting director Kharmel Cochrane texted Lily-Rose Depp in advance to apologize for dismissing her as a serious contender for Ellen Hutter, doubting she could act and expecting nepotism criticism over her parents Vanessa Paradis and Johnny Depp, then changed her mind once Depp auditioned and proved her wrong.
- Robert Eggers gifted Lily-Rose Depp the prop of Ellen's locket after filming, while Nicholas Hoult kept and framed the prosthetic penis worn by Bill Skarsgård as Count Orlok, one of several custom props actors took home.
- The production featured around 5,000 trained rats, and cast Romanian TikTok personality Bunica Gherghina as the elderly lady at the Transylvanian inn.
- Prosthetic makeup effects designer David White said Count Orlok's physique was partly influenced by Ötzi, the natural mummy discovered in the Ötztal Alps in 1991, because Eggers wanted the look of a body with every last drop of blood sucked from it.
- Robert Eggers considered Daniel Day-Lewis and Mads Mikkelsen for Count Orlok before Bill Skarsgård was cast in September 2022; Skarsgård had originally auditioned for and been offered the role of Thomas Hutter.
- Willem Dafoe joined the cast in January 2023, reuniting with Eggers after The Lighthouse and The Northman; he had previously played Max Schreck/Count Orlok in Shadow of the Vampire (2000) and had been set to play the vampire again for Eggers.
- Anya Taylor-Joy, reteaming with Eggers after The Witch, was cast in August 2017 and was still attached in 2020; Harry Styles had been cast to star opposite her but dropped out of the project in 2021 citing scheduling concerns.
