Plot summary
Kate Fitzgerald, the older daughter of Sara and Brian, has acute promyelocytic leukemia. No one in the family, including her brother Jesse, is a genetic match, so her parents conceive Anna by in vitro fertilization to be a donor. Anna has given blood and stem cells since she was born. Sara takes her donations for granted; Brian is not sure. When Kate is fifteen her kidneys fail, and Anna, at eleven, is expected to give a kidney. With Jesse's help she asks the lawyer Campbell Alexander to win her medical emancipation.

- Sisters Dakota and Elle Fanning were originally cast to play Kate and Anna, until Dakota learned that she would be required to shave her head for the role. She dropped out of the film, as did Elle.

Jodi Picoult said in an interview that she regretted letting Nick Cassavetes direct the film, as he changed the ending from her novel. In the book Anna gains medical emancipation but is killed in a car crash on the way home from court, and her kidney saves Kate's life.
