Plot summary
Greg Gaines is a seventeen-year-old senior at a Pittsburgh high school who keeps away from every clique and forms no close friendships. His only companion is Earl, who he insists is just a coworker; at home the pair make brief parodies of well-known movies. When Greg's parents learn that Rachel Kushner, a girl he knew in Hebrew school, has leukemia, they require him to befriend her. Their first meeting, on her staircase, is awkward for both of them, but a conversation about her pillow collection gets them talking. Greg brings Earl along and the two let her watch the films they have made. As Rachel begins chemotherapy and loses her hair, Greg spends less time on schoolwork and more of it with her.

- Filmmakers Edward Bursch and Nathan O. Marsh made 21 shorts, a mix of stop-motion animation and live action, to stand in for the film parodies Greg and Earl shoot, among them the final short made for Rachel and set to Brian Eno's "The Big Ship".
- The Criterion Collection lent its own library of classic films so the book-and-DVD store seen in the film could be stocked with them.
- Writer Jesse Andrews, a Pittsburgh native, had his own family home in Point Breeze used as Greg's house in the film.
