Plot summary
Dr. Rodney Barrett practices in Thornton, California, where the deaths of his wife Alice and her sister Nancy have cost him the trust of the town, and only one patient still consults him. He lives with Marge, who is three, and her nanny, Miss Kushins, and plans to marry Sylvia Stevenson, a young widow. Polly Baron, who works as his nurse, would rather he take Marge and leave. Then Marge disappears from the house. Polly answers a call from a man who says he has taken the girl and buried her alive, and who hints that the child keeps company with the dead. Barrett and Polly think she may lie in a used grave and search the graveyard and Ed Quigley's funeral parlor, where a child's coffin has gone missing.

- Macabre was one of William Castle's first uses of the promotional gimmicks that later made him famous: every customer received a certificate for a $1,000 life insurance policy from Lloyd's of London in case they died of fright during the film.
- To promote Macabre, William Castle hired fake nurses to stand in theater lobbies and parked hearses outside, arrived at the premiere by emerging from a coffin, and at a Minneapolis theater sealed himself in a coffin like the kidnapped child in the story.
- William Castle mortgaged his Beverly Hills house to finance Macabre, the first of five films he planned to make with Robb White through their company Susina Associates over the following sixteen months.
