Plot summary
Cathryn writes children's books in London and is expecting a baby. She is at work on a fantasy story and gathers photographs for its illustrations. A female voice breaks into her telephone calls and hints, mockingly, that her husband Hugh is seeing someone else. Hugh comes home to find her shaken, and puts the episode down to strain and her condition. He takes her to a cottage in the Irish countryside, her childhood home, so she can work in quiet. There she hears her name spoken and watches Hugh cross the kitchen and turn into Rene, a lover of hers who died in a plane crash. Rene keeps appearing and talking with her. A neighbor, Marcel, a former lover, arrives with his young daughter Susannah, and Cathryn begins to lose track of which man she is looking at.

- The characters' names in the film are the inverses of the real-life names of the actors playing their counterparts: the protagonist Cathryn is played by Susannah York, while actress Cathryn Harrison plays a character named Susannah.
- Altman sought Susannah York for the lead after seeing her performance in Jane Eyre, and she accepted once he revised the screenplay. York nearly backed out after discovering she was pregnant, and was dressed in loose-fitting clothing to conceal it during the shoot.
- The children's story "In Search of Unicorns", which Cathryn reads in voice-over against shots of horses, clouds and waterfalls, was actually written by Susannah York herself.
