Plot summary
After Frank Cotton killed her father, Kirsty Cotton is taken to a psychiatric hospital and tells Dr. Channard, the doctor in charge, and his assistant Kyle MacRae what she witnessed, insisting her father is trapped in Hell and asking that the bloodstained mattress her stepmother Julia died on be destroyed. Only Kyle believes her account. Channard has his own obsession with the Lament Configuration and has the mattress brought to his house. Kirsty meets Tiffany, a young mute patient who solves puzzles with unusual skill, and at night a skinless figure appears in her room and writes a plea in blood.

SPOILER
Julia was meant to survive Hellbound as the head embedded in the Hell Pillar, but Clare Higgins did not want to return for more sequels and asked for her character to be killed off instead.- Hellbound was initially rated X by the Motion Picture Association of America, and six minutes were cut to qualify for an R. Clive Barker attributed the rating to his preference for showing the grotesque explicitly rather than hinting at it.
- Nicholas Vince, who plays the Chatterer, took a hook to the jaw while filming a scene in which his character is impaled on a swinging torture rack surrounded by hanging chains.
- A scene with Doug Bradley as a normal doctor was filmed but dropped from the final cut, as the filmmakers feared confusing viewers and the effects were poor. Set photos of Pinhead and the Female Cenobite dressed as surgeons still appeared on VHS and DVD covers, starting rumors of an "infamous deleted surgery scene".
- Kenneth Cranham said he took the part of Dr Channard because his grandson, a fan of the original, pestered him into accepting it; Francis Matthews was also offered the role.
- Composer Christopher Young returned for a bigger, more bombastic score, and had the morse code for the word god worked into the horn-like sound supposedly coming from Leviathan in Hell's labyrinth.
- The script originally brought back Kirsty's father Larry Cotton, but Andrew Robinson turned down the role; reported reasons included a disagreement over fees and clashing schedules, though none was ever confirmed.
- A more elaborate backstory for Pinhead was scripted for Hellbound but was cut because the film had run out of funds after its budget was slashed.
