Plot summary
Since he was a boy, food has been the comfort Dominick DiNapoli reaches for, the thing his mother offered when he was unhappy, and his cousin Sal, who is also obese, shares his appetite for snacks. When Sal dies at 39, Antoinette, Dominick's sister, urges him to visit a diet doctor so he does not ruin his health the same way, and Dominick agrees once he notices the signs in himself. The plan's long list of forbidden foods defeats him, and Antoinette signs him up for the Chubby Checkers support group. Meanwhile, Dominick meets Lydia, a thin diabetic who runs the neighborhood antiques shop. They have plenty in common beyond food, but he is self-conscious about his body and cannot bring himself to ask her out.

- Anne Bancroft wrote and directed Fatso, and it is her only such credit: the actress, far better known in front of the camera, never wrote and directed another film. It starred Dom DeLuise, Ron Carey and Candice Azzara.
- The film is notable for Brianne Murphy, who by photographing it became the first woman to be credited as a cinematographer on a studio film.
- Fatso was also the first film that Brooksfilms ever produced: the production company was run by Bancroft's husband, Mel Brooks, and this was the picture it started out with.
- The film marked a turning point for actors Richard Karron and Paul Zegler, who played members of DeLuise's obese "Chubby Checker" support group. Both of them lost large amounts of weight in the years after making it.
