Plot summary
Five friends, Jeff, Marcy, Paul, Karen and Bert, rent a remote cabin in the woods to celebrate October break. Bert goes out to hunt squirrels and shoots a disfigured, bloody stranger, then keeps the shooting to himself. That night a friendly drifter named Grimm and his dog, Dr. Mambo, join them at the campfire and leave when the rain comes. The stranger, the hermit Henry, stumbles into the camp pleading for help, and Bert closes the door on him. Paul calls Deputy Winston, who promises to send a tow truck for their truck. When Paul tries to comfort Karen and finds a bloody wound on her thigh, the group puts her in a shed.

- Eli Roth wanted Cerina Vincent to bare her buttocks in her Cabin Fever sex scene with Rider Strong, but she refused and warned he would have to recast; they compromised on exactly one inch, which Roth measured before bedsheets were taped to her backside.
- Auditions for the role of Marcy were scheduled for September 11, 2001, and Eli Roth and the producers tried to cancel them; the chaos of the attacks made it impossible to reach many of the actresses due to try out.
