Plot summary
Paul lives in Marshall, North Carolina, where he repairs cars for his uncle Leland. His friends Tip, Bo and Bust-Ass know him as a ladies' man, and nearly every girl in town has been with him; his romances usually last only a few weeks. His mother, Elvira, dresses as a clown to cheer children at the hospital. Then Noel, Tip's teenage sister, comes back from boarding school and takes a job at a factory in Asheville. She is more thoughtful than the girls Paul knows, and the two begin seeing each other in private. When word reaches Tip, he confronts Paul and demands to know whether the two are sleeping together.

- Before he partnered with Jean Doumanian and Lisa Muskat on All the Real Girls, David Gordon Green was offered a larger budget by a studio, but passed because the studio wanted Freddie Prinze Jr. cast in the lead instead of Paul Schneider.
- David Gordon Green avoided genre clichés and aimed instead for "unique magical moments"; the result was that the first forty pages of the script, including the scene in which Paul and Noel meet for the first time, were thrown out of the finished film.
- When funding fell through for the science fiction film and western he planned after George Washington, David Gordon Green took a job at a doorknob factory before turning back to All the Real Girls, which he felt "obligated" to make while those feelings were still fresh.
- Danny McBride, who had been a second unit director on George Washington, was cast in a supporting role in All the Real Girls, which was his acting debut.
