Plot summary
Anna is troubled by the way her family shuns her brother Bobby, a veteran of World War II who returned home shell shocked. She becomes involved with Billy, a local mob boss. Newspapers and Life magazine promote the transorbital lobotomy as the new vogue in American medicine, and when Dr. Harold Ashton arrives in town to perform the icepick procedure on alcoholics, veterans and other troubled outsiders, Anna fixates on having one. Alarmed, Billy sends her to a clinic run by Tom, a Korean War veteran in his pay who pretends to be a neurologist. Tom asks her to put off the operation and come see him that night.

- The idea for A Hole in One was born out of a performance piece Ledes had staged at the American Fine Arts Gallery in SoHo in the early 1990s, based on the records of a WWII veteran who had experienced a psychotic break and for whom a lobotomy had been recommended.
- Ledes conducted extensive research for the film over many years, including volunteering at an outpatient center for the severely mentally ill and visiting George Washington University, which holds the archives of Dr. Walter Freeman.
