Plot summary
After the existence of vampires and Lycans becomes public knowledge, a program meant to study their powers turns into an extermination campaign called the Purge. Selene and Michael are caught, and Selene is placed in cryogenic suspension. Twelve years later she escapes, and learns that a second subject has also fled the facility. Visions from another person's point of view pull her onward. David, a vampire, has been following her. Weakened Lycans are hunting the same runaway, and Selene has to fight through their pack to reach the fugitive.

- Underworld: Awakening began principal photography in March 2011 at Simon Fraser University in Vancouver without a finished script, after the story had already gone through rewrites by a number of writers and different versions.
- Producer Richard Wright said production on this film was more difficult than others: Len Wiseman, busy at the same time with the Total Recall remake, was often called for help and sometimes rewrote a scene the night before it was filmed.
- Directors Måns Mårlind and Björn Stein said the ending in the finished film was written during filming and is not the one they had when production started, but they gave no details of the original ending.
- Scott Speedman, who played Michael Corvin in the first two Underworld films, announced he would not reprise the role in the fourth; he appears only in archive footage, and a stand-in was used for the scenes where Michael is unconscious.
- An alternate opening, dropped from the finished film, had an escaped mutated Lycan destroy Selene's cryo tank and stalk a weakened Selene around the lab; it may have been changed because the scene was difficult to cut around nudity.
- Underworld: Awakening is one of the first films to be shot using RED EPIC digital cameras in 3D.
