Plot summary
Beneath Raccoon City, the Umbrella Corporation runs the Hive, a genetic research facility. When a thief releases the T-virus inside it, the Hive's artificial intelligence, called the Red Queen, seals the complex and kills the staff to keep the outbreak from reaching the surface. Alice comes to in a deserted mansion's bathroom, unable to remember who she is, and a stranger tackles her as commandos led by James Shade break in. The stranger says he is Matt Addison, recently transferred to Raccoon P.D. The commandos take Alice and Matt down into the Hive, where Spence, another amnesiac, is hiding on their train. The team explains that everyone but Matt works for Umbrella, and that Alice and Spence were posted to guard the Hive's hidden entrance while pretending to be married.

- In 1998 George A. Romero directed a television commercial for the video game Resident Evil 2 that was shown only in Japan, but it impressed Sony enough for them to ask him to write and direct Resident Evil; the original game's director, Shinji Mikami, was a fan influenced by his films.
- David Boreanaz was intended to portray the male cop lead of Matt Addison, but he turned down the role to continue work on the WB series Angel.
- Developed as a prequel set in the same continuity as the video game series, the film was initially titled Resident Evil: Ground Zero, but was retitled after the September 11 attacks.
- Rather than ordinary background actors, the production hired professional dancers to star as the film's zombies, because they had better control of their body movements.
- The film originally received an NC-17 rating and Anderson had to make cuts to it; the uncut version was teased but has never been released.
