Plot summary
Martin Lomax is a short, asthmatic man who shares a flat with his mother in East London. He works nights in a parking garage tollbooth, watching the first Human Centipede film on a laptop. His psychiatrist, Dr. Sebring, prescribes heavy medication and behaves inappropriately toward him. Martin's mother blames him for his father's imprisonment and is emotionally abusive. Martin becomes fixated on building a centipede of twelve people, using crude techniques rather than surgical tools. He acquires a warehouse and begins abducting people for the centipede. Those he captures include a young couple, a husband and his pregnant wife, and two women who had been drinking.

- In June 2011 the British Board of Film Classification refused to classify The Human Centipede 2 (Full Sequence) for a direct-to-video release, effectively meaning that the film could not legally be supplied in any format in the UK.
- Mark Hamill was offered an undisclosed role in the film, but turned it down as he was "disgusted" by the premise.
- In April 2016 a Tennessee high school teacher was suspended after the film was played during class. Tom Six responded by tweeting that it "should be mandatory to watch THC2 in school classes" because it deals with a character who is bullied, and said he would give the teacher an autographed copy.
- In October 2011 the BBFC ultimately granted the film an 18 certificate for "very strong bloody violence and gore, and sexual violence" after 32 compulsory cuts were made, totalling 2 minutes and 37 seconds.
- In May 2011 the uncut film was granted an R18+ classification by the Australian Classification Board, but the decision was overruled after Minister for Justice Brendan O'Connor asked for a review; on 28 November 2011 a three-person board unanimously refused it classification.
- On 13 December 2011 the Australian Classification Board classified a modified version at R18+ for "high impact themes, violence and sexual violence". Monster Pictures said thirty seconds had been "modified" and that the decision highlighted "the absurdity of Classification Review Board's decision to ban the film in the first place."
