Plot summary
In Miles County, New York, on Halloween night, Tara and her friend Dawn walk out of a party and run into Art the Clown, who trails them to a pizzeria, where he is thrown out after smearing the bathroom with feces. Dawn then finds that a tire on her car has been slashed, so Tara calls her sister Victoria to come and get them. While they wait, Tara persuades Mike, a pest exterminator working in a nearby apartment building, to let her inside to use a restroom, and she meets a squatter known as the Cat Lady, who carries a doll she treats as her infant child.

- Actress Catherine Corcoran sued producer Phil Falcone and director Damien Leone in October 2025, saying they paid her only $8,341 of the one percent of profits she was promised for starring in Terrifier, its sequels and its merchandise.
- Corcoran's lawsuit also accuses the filmmakers of sexual harassment, including distributing sexually explicit material without her consent, and says she never gave the prior authorization SAG-AFTRA rules require for nude scenes before being made to hang nude upside down for ten hours.

Catherine Corcoran, who played Dawn, filmed the hacksaw death hung upside down and shackled by her feet without a rig, unable to even out her weight, so shooting was done in 40-second increments with a platform placed beneath her between takes.- Mike Giannelli, who played Art the Clown in Leone's earlier shorts and All Hallows' Eve, did not return for the feature, and the role went to then-unknown David Howard Thornton, cast after he improvised a kill scene in mime at his audition.
- Damien Leone's attempt to raise money for the Terrifier feature through an Indiegogo campaign fell short of its goal, and filmmaker Phil Falcone then contributed the funds to make the film in exchange for a producer credit.
- Jenna Kanell, who played Tara, had first auditioned for Leone's short film The 9th Circle (2008) and did not get the part; she kept in touch with him, and he cast her in Terrifier in 2015 based on that earlier audition.
