Plot summary
Approaching sixty and long retired, Rocky Balboa lives in Philadelphia as a widower, four years after his wife Adrian died of cancer. He runs a small restaurant named after her, telling customers stories of his old fights. His son Robert works as an accountant, and their relationship is strained; Paulie, his brother-in-law, accuses him of living in the past. Rocky meets Marie, whom he once walked home thirty years earlier, and her teenage son Steps, and he takes an interest in them. In the boxing world, Mason "The Line" Dixon holds the heavyweight title but is mocked for never having faced a true contender. A simulation pits a younger Rocky against Dixon and ends in a disputed knockout in Rocky's favor, and Dixon's promoters propose a charity exhibition.

Can contain spoilers, watch the movie first.
- Stallone initially wanted Roy Jones Jr. to portray Dixon, but after Jones did not return his phone calls he tapped Antonio Tarver to fill the role. Tarver then accidentally knocked out Stallone during the filming of one of the segments of the fight.
SPOILER
Stallone chose not to have Talia Shire reprise her role as Adrian, telling USA Today that in the original script she was alive but it just did not have the same dramatic punch. Shire said the film has great regard for the process of mourning and uses it to empower Rocky.- Sportscaster Stu Nahan, part of the ringside commentary team for all the bouts in the first three films and the Creed and Drago fight in Rocky IV, provided the commentary for the computer-generated fight between Dixon and Balboa. He was diagnosed with lymphoma during filming and died on December 26, 2007.
- Mike Tyson, who had retired by the film's release, makes a cameo appearance taunting Dixon as the fighter enters the ring, and real-life boxing promoter Lou DiBella portrays himself as Dixon's promoter.
- Stock footage from actual Tarver fights, as well as footage from Dixon's previous fight shown at the beginning of the film, was used as clips for Dixon's part of the teaser.
