Plot summary

Months after the Dead Moon Circus, Usagi Tsukino and her friends live as ordinary students in Tokyo. Mamoru Chiba, her boyfriend, is about to leave for the United States, and at the airport he gives her a ring; Sailor Galaxia then takes his Golden Crystal and vanishes. Usagi convinces herself that he caught his flight. After saying goodbye to Chibiusa, who goes back to the 30th century, she and her friends watch the idol trio Three Lights perform. Sailor Iron Mouse, part of Shadow Galactica, an empire that hunts Sailor Crystals across the galaxy, attacks the concert, and the Sailor Starlights, guardians from another star system, drive her off; Usagi and her friends recognize them as the Three Lights. A small child named Chibi-Chibi then appears at Usagi's home, and the girls wonder if she is related to Chibiusa.

Keiko Kitagawa, who played Rei Hino/Sailor Mars in the 2003 live-action series, was cast as Sailor Cosmos after the director and producers decided the part should not go to Kotono Mitsuishi; she first felt it should be her former co-star Miyuu Sawai's, but took it after her former castmates unanimously urged her.
Kotono Mitsuishi, the voice of Usagi Tsukino/Sailor Moon, confirmed that Sailor Moon Cosmos would mark her final performance as the character; the returning cast had already recorded their lines for the first film before the announcement.
Screenwriter Kazuyuki Fudeyasu said Sailor Moon Eternal was about friends coming together one by one, while Cosmos was the opposite, losing friends one after the other, and he added a scene before the credits as an homage to the very first chapter of the manga.
Director Tomoya Takahashi was highly recommended by Studio Deen staff during the COVID-19 pandemic, and producers Hideharu Gomi and Seira Takano gave him more freedom than previous directors, which he used to include homages to the Sailor Moon manga, the 1990s anime and the 2003 live-action series.
