Plot summary
In Pittsburgh, the vampire Marie feeds on criminals; her latest mark is mafioso Tony Silva, whom she bites and then shoots with a shotgun, hiding the bite marks on his neck. Undercover cop Joseph Gennaro is infiltrating the crime family of Sal Macelli until the media reports his presence at a crime scene and the district attorney places him in protective custody. Marie then arrives at Sal's mansion as a dinner guest. He serves garlic mussels, which ward her off, and when he attempts to rape her she fights him off and drains his blood, and Lenny, his driver, forces her to flee. Gennaro follows the blood to a church, where he comes across Marie.

- The MPAA wanted to give Innocent Blood an NC-17 rating, so John Landis cut out two minutes of sexual and violent content to secure an R; in post-production he had already made about 15 to 20 minutes of cuts to the film.
- John Landis cast Anne Parillaud, a former ballerina, as the vampire Marie after seeing her in La Femme Nikita, wanting a Marie who was beautiful, sexy and sympathetic; he refused to dub her French accent even though preview audiences had trouble understanding her.
- Landis had planned to shoot in Philadelphia and set the film in New York, but chose Pittsburgh after visiting it; the city's Little Italy, in the Bloomfield neighborhood around Liberty Avenue, appears in many of the film's outdoor scenes.
