Plot summary

Sarah takes a job at Bright Sky Homes in Liverpool, where she has a gift for reaching the residents. One of them is Tony, a middle-aged man with early-onset Alzheimer's disease who often leaves the building to walk to his mother's house, forgetting that she has died. The shifts are hard. In March 2020 the pandemic reaches the UK, and the home has an outbreak among its residents while protective equipment is scarce because NHS hospitals are supplied first.

The sequence in which Jodie Comer's Sarah is alone on the night shift, desperately trying to reach the emergency services, was shot in a single take lasting over 26 minutes; Comer said it was probably the most difficult scene of the whole shoot.
Jack Thorne said Stephen Graham came to him about two years earlier with an idea for a script for himself and Jodie Comer; Thorne got nothing until the pandemic happened and care homes were getting "squashed and battered by the government".
Jack Thorne's original intention was a story about different aspects of care homes and care home politics, but he was told to change it by The Forge founder George Faber.
On 8 May 2022 Jodie Comer won the British Academy Television Award for Lead Actress for playing Sarah, her second win in that category, and thanked caregivers of the Dementia Society in her speech.
Jack Thorne was disappointed that the film did not shift public opinion; in 2025 he said care work is still badly treated and that he had wanted Help to be Cathy Come Home but it didn't achieve that.
