Plot summary
A car goes off a bridge into the river with Trevor Gooden and his wife Kirsty Cotton inside. Trevor escapes alive; police divers who search the water find both doors open and no trace of Kirsty. A month later he wakes in a hospital. A head injury has left his memory uncertain, so that he cannot tell fantasy from reality, and a murder case makes him its prime suspect. He has numerous encounters with the homicide detectives Givens and Mike Lange. Hallucinations come to him, and events he takes as real turn out to be figments of his imagination.

- Producers spent all of pre-production unable to reach Ashley Laurence because her agents would not give out her contact information, so her character was rewritten but still kept the name Kirsty as an easter egg for fans; Doug Bradley then offered to contact Laurence himself and she agreed to return.
- Without the approval of Dimension executives, director Rick Bota gave a screening of the film's workprint to Clive Barker, who gave notes on how to improve the third act; it was the last time Barker was involved in the direct-to-video Hellraiser sequels.
- Bota let Doug Bradley rewrite the dialogue for Pinhead and Kirsty's reunion, including references to the first two films, but most of those lines were cut because Bota did not want to confuse new viewers.
