Plot summary
Loomis Crowley, Rex and Sarah play an underground game called Stay Alive, and all three are found dead in the ways their characters died. At the funeral, Loomis's belongings go to his friend Hutch MacNeil, the game among them. Hutch tries it, and October, her brother Phineus, Sarah's friend Abigail, Swink and his boss Miller join in, Miller from his office. The game is a survival horror played online, set in an abandoned mansion on Gerouge Plantation. Its opening screen carries the Prayer of Elizabeth, which asks that all who resist perish so their blood keeps Countess Bathory young. October, who knows the occult, says wild roses drive off undead spirits, and the players pick them while heading for a mausoleum and a tower.

For a planned sequel the filmmakers explained how Jimmi Simpson's character Phineas would return: "he would have been this ghost in the machine", trapped in the game, found to have been there the whole time, and acting as mentor to the movie's new characters.- In August 2022 the filmmaker Bell revealed that developments for a potential sequel were ongoing, with creatives "trying very hard to make another one"; in March 2026, the film's 20th anniversary, Bell and Peterman still showed interest while admitting it seemed unlikely.
- Stay Alive was made available on DVD in two versions, an unrated edition running 100 minutes and a "PG-13" edition running 85 minutes; the 15 minutes of new unrated footage include a new character and subplot plus more adult material.
- As of December 2011 the total amount of DVD units sold was 874,827, bringing in $13,636,869 in revenue for the film.
- Released in March 2006 in the United States by Buena Vista Pictures Distribution, Stay Alive met negative reviews from critics but still grossed $27.3 million against a $20 million budget.
- The critical drubbing did not stick: despite its initial negative reception, Stay Alive has been regarded by some in the years since as a cult classic horror movie.
