Plot summary
Riley McKendry is a recovering addict who shares a home with her brother Matt, Matt's boyfriend Colin, and a roommate named Nora. Riley and her boyfriend Trevor discover a mysterious puzzle box in an abandoned storage warehouse. After Matt accuses Riley of relapsing, she storms out. In an empty park, she solves the box but the blade misses her, and the Cenobites, mutilated humanoids from another dimension, appear and demand a sacrifice. Matt finds her incapacitated and, while trying to wake her, accidentally cuts himself on the box. He goes to a restroom, then vanishes. Riley and Trevor believe the box caused his disappearance and set out to learn what has happened.

- Producers auditioned drag performer Gottmik for the role of Pinhead after Gottmik showcased a Pinhead-inspired look on RuPaul's Drag Race, as revealed in a July 2021 Entertainment Weekly interview.
- Julien Maury and Alexandre Bustillo vacated the Hellraiser remake and moved onto Halloween II after clashing with producer Bob Weinstein, who wanted a film with wider mass appeal; Marcus Dunstan and Patrick Melton were tapped for a page-one rewrite.
- While working on the reboot, Lussier and Farmer pitched a prequel starring William Fichtner as Pinhead and another version in which Pinhead was portrayed by a woman.
- Bruckner and his crew reached out to Bradley to make a cameo appearance in the film, but he declined, citing potential COVID-19 complications and his desire to leave his Pinhead performance's legacy intact.
