Plot summary
El Indio, a bank robber, is broken out of prison when his gang overpowers the guards and leaves one alive to spread word of what they can do. He carries a musical pocket watch with a photograph inside its cover and plays it before a gunfight. Two bounty hunters ride for the price on his head: Manco and Colonel Douglas Mortimer, a former army officer. Both come to El Paso, where word says El Indio means to rob the bank, whose disguised safe holds almost a million dollars. Manco reaches town behind Mortimer and watches him provoke Wild, a hunchback gunman scouting the bank.

- Clint Eastwood would not commit to a second film before seeing the first, so the filmmakers rushed him an Italian-language print of A Fistful of Dollars. He agreed after a debut screening at CBS Production Center convinced the audience.
- Charles Bronson was again approached to star in the sequel but thought its script too similar to the first film, so Lee Van Cleef accepted the role instead.
- Ennio Morricone began writing the score before production started, under Sergio Leone's explicit direction, because Leone often shot to the music on set.
- Screenwriter Luciano Vincenzoni wrote the film in nine days, but Sergio Leone was dissatisfied with some of its dialogue and hired Sergio Donati as an uncredited script doctor.
- Production designer Carlo Simi built the film's town of El Paso in the Almería desert, and it still exists as the tourist attraction Mini Hollywood.
- All footage was shot without sound, so Clint Eastwood and Lee Van Cleef returned to Italy to dub their dialogue. Van Cleef used his native New Jersey accent although Colonel Mortimer is stated to be from the Carolinas.
- Journalist Kim Newman said the film changed how audiences viewed bounty hunters, moving them from a profession ranked lower than a card sharp to one of heroic respectability.
- Maurizio Graf sang a vocal version, "Occhio Per Occhio"/"An Eye For An Eye", of the cue "Sixty Seconds to What?". His performance did not appear in the film but was released as tie-in 45 rpm records.
