Plot summary
Five years after leaving Earth, the Antareans come back for the cocoons left in the ocean, which an earthquake may endanger. Their human friends travel with them: Art and Bess, Ben and Mary Luckett, and Joe and Alma Finley. Ben and Mary visit their family and their friend Bernie Lefkowitz, who has found love with Ruby Feinberg. Art and Bess learn that Bess is pregnant and plan to raise the child on Antarea. Joe learns his leukemia has returned, though leaving Earth would cure it. Back on Earth the group feels their old weaknesses again. A research team has taken one of the cocoons to a laboratory for testing, and the visitors need it back before their ship returns for them.

- Brian Dennehy initially refused to reprise his role as the alien leader Walter, eventually agreeing only to a three-minute scene at the very end of the film, which he did unpaid, purely as a favor to his former co-stars.
- Because Dennehy was tied up starring in a New York stage production of The Cherry Orchard, he shot his scenes in a few days in Florida and later in Los Angeles after the rest of filming had wrapped, and was bluescreened into the footage he needed to appear in.
- David Saperstein, who wrote the original story for the first Cocoon, wrote a treatment for this sequel, but the studio rejected it, so he published his own version instead as a novel called Metamorphosis: The Cocoon Story Continues.
